Elderberry research is less developed. A 2021 systematic review identified five randomized trials examining prevention or treatment of viral respiratory illness. The review found that elderberry might reduce symptom duration or severity in some circumstances, but it did not support claims that elderberry prevents respiratory infections.

Different elderberry products cannot be treated as interchangeable. Extract type, anthocyanin content, preparation, serving amount, added ingredients, timing, and participant population vary. Positive findings for a particular syrup cannot automatically validate every gummy or capsule.

The evidence supports balanced language: vitamin C and zinc contribute to normal immunity, while elderberry remains a traditional botanical with limited clinical research. None should be promoted as a cure or guaranteed defence.
